FREE & REDUCED MEALS
If you feel that your family might qualify for the Free/Reduced
Meal Program, please be sure to fill out an application and return
it to the Superintendent’s Office as soon as possible. These forms
may be obtained from your child’s building office or at the Central
Office. Once the form has been processed and your family’s
qualification level has been determined, a letter will be mailed
home to inform you as to whether or not you have qualified for the
Free/Reduced Meal Program. The Free/Reduced Meal Application covers
all of the students in your household, so please be sure to only
send in one form per family.
To be considered complete, the form MUST include the
following:
•
All students in the household attending Marshfield schools
•
10 digit DSN Number from your Food Stamp Award Letter, if
applicable(not the16 digit number from your EBT card)
•
Names of all people living in the household
•
Gross income of anyone living in the household and how often that
is paid
•
Name, Social Security Number and signature of adult filling out the
form
Without all of this information, we cannot process the form and we will be required to return the form to you for completion, delaying the receipt of your child’s benefits. If you have any questions, please contact Nedra Fillmer or Jennie Ruth at (417)859-2120.
Note: If you include the 10 digit DSN number from
your Food Stamp Award letter, you do not need to include your
income information. It is still necessary to include your social
security number and signature on the form to be considered
complete.
